    PARTICIPANT RELEASE OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT

    In consideration of being allowed to participate in any way in the training, martial arts instruction, sparring, open mats, tournaments, events, and related activities provided by Dalua BJJ LLC (hereafter referred to as "the Academy"), the undersigned participant (or parent/legal guardian if participant is under 18), acknowledges, appreciates, and agrees to the following terms:

    1. ASSUMPTION OF INHERENT RISKS
    I fully understand that Brazilian Jiu-Jitsu, submission grappling, martial arts training, physical conditioning, and related activities are inherently dangerous full-contact sports. I acknowledge that these activities involve substantial risk of severe bodily injury, including but not limited to: sprains, fractures, joint dislocations, torn ligaments/tendons, neck/spinal injuries, head trauma, skin infections (staph, ringworm, MRSA), permanent disability, or death. I voluntarily, knowingly, and freely assume all risks, both known and unknown, associated with my (or my minor child's) participation, even if arising from the negligence of the Releasees (defined below) or others.

    2. WAIVER AND RELEASE OF LIABILITY
    I, for myself (and on behalf of my minor child, heirs, assigns, personal representatives, and next of kin), HEREBY FOREVER RELEASE, WAIVE, AND DISCHARGE Dalua BJJ LLC, its owners, instructors, employees, contractors, agents, landlords, and other participants (collectively, "Releasees") from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death or property damage) sustained while participating in activities, using the facilities, or present on the premises.

    3. INDEMNIFICATION AND HOLD HARMLESS
    I further agree to INDEMNIFY AND HOLD HARMLESS the Releasees from any and all liabilities, loss, damage, costs, or legal fees that they may incur as a result of any claim, lawsuit, or demand brought by me, on my behalf, or on behalf of my minor child as a result of participation at the Academy.

    4. PHYSICAL HEALTH AND MAT HYGIENE ACKNOWLEDGMENT
    I certify that the participant is in good physical condition, has no undisclosed medical conditions or physical limitations that would make participation unsafe, and has not been advised otherwise by a medical professional. I agree to abide by all Academy rules, including mandatory hygiene standards (clean gi/training gear, clipped fingernails/toenails, refraining from training with open wounds or communicable skin infections). I agree to immediately cease training and inform an instructor if I (or my minor child) experience dizziness, unsafe conditions, or severe distress.

    5. MEDIA AND MODEL RELEASE
    I grant Dalua BJJ LLC the right to record, photograph, and film the participant's image, likeness, and voice during attendance or participation. I agree that the Academy may use these materials for marketing, promotional materials, website content, and social media without compensation or notification.

    6. SEVERABILITY AND GOVERNING LAW
    This Agreement shall be governed by and construed under the laws of the State of Texas. If any provision of this agreement is found to be illegal, invalid, or unenforceable by a court of competent jurisdiction, the remaining provisions shall remain in full force and effect.

    I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

    IF SIGNING ON BEHALF OF A MINOR: I certify that I am the parent or legal guardian with legal responsibility for this participant. I consent and agree to their release and assumption of risk as provided above, and agree to indemnify and hold harmless the Releasees from all liabilities incident to my minor child's involvement.
